We are informed that members who have missed the October 4, 2013 deadline for the Dependent Eligibility Verification Audit (DEVA) may still submit required documents by November 4, 2013 in order to prevent termination of coverage for their dependents.  Documents must be submitted via fax or secure online upload.  Submission by U.S. Mail is no longer acceptable.

Members may submit documents via secure fax at 1-877-965-9555.

Members may also submit documents via secure online upload by logging on to  Member login instructions are as follows:

  • Login Name - Your Dependent Verification ID. (Example 1234567)

Your Dependent Verification ID can be found at the bottom center of the City’s letter. Note that you must use this number. It is NOT your Employee ID (EMPLID) or your Social Security Number. It is a number to be used specifically, and ONLY, for this process.

  • Password - Your password is the last four digits of your Social Security Number.

You will be instructed to change your password upon entering the secured site.

Members who have submitted documentation prior to the original October 4 deadline should be reminded that, even if the documentation is deemed insufficient, their dependents will not lose coverage during the appeals process initiated by the member.  We will brief you on the appeals process as we receive further details from the City.
