The City’s Health Benefits Program annual Fall Transfer Period (“Open-Enrollment”) will take place from November 1, 2019 through November 29, 2019 for active police officers.
During the Open-Enrollment period, members are able to make changes which are not normally permitted during the year. Members are able to change plans, add or drop a rider, and/or add a dependent(s). To make changes, you must complete a Health Benefits Application and submit your completed form to:

You can download a Health Benefits Application from the City’s website at www.nyc.gov/hbp. We are informed that you may also use the NYCAPS online portal to make changes to your City health coverage. For information concerning online access please call NYCAPS Central at (212) 487-0500.
Changes made during this year’s Open-Enrollment period will become effective in January 2020.
